Always use version control (e.g. CVS)
Use a problem ticket system with a defined process (e.g. Bugzilla)
Log all version changes against a ticket number, enforce if possible.
Use an automated build program (e.g. Make) and store the build script and build output against the problem ticket(s). Also record the build environment
Use frequent baselines for further work
